Registration for V-O on Stage

PLEASE READ CAREFULLY: 

• If you are in two acts, pay only once.

• Emcees - If you also are performing, please do a separate registration for Emcee but do not pay twice.

• You MUST fill in the name and email of the parent/guardian for each participating child. Please DO NOT leave this blank.

• Please complete all items before submitting (for example, the name/email of the parent/guardian for each participating child, each child's tee shirt size, etc.)
